He takes the powder out of the canister it comes in and pours it in a generic canister likes he’s been doing for years, and then mislabels it, rifle powder as opposed to shotgun powder. There you have it, 50,000 psi. Then he shoots the first shell of that mislabeled batch, gun blows. As the old saying goes “familiarity breeds contempt.”

MEC charge bars have had holes in the powder bushing bore for a long time. It's difficult to use the machine without a bushing.

The head needs to be tipped back to change powder bottles. No powder should remain in the bushing. Generally, a powder change means a bushing change also. Not always.

Personally, I stick to middle of the road recipes. Nothing real low or real high pressure. I do prefer Alliant powders as I consider them the easiest to ignite of the bunch.

I've suspected for a long time that quality control on primers is not what it should be. That may be the joker in the deck.

Only other information I found was he was at station 5 or 6 and the top barrel landed the other side of station 8.

Same can happen with a shotgun.

So, my belief is this.

I cringe when I read about handloaders toying with untested recipes.
As to mistakenly loading a shell or two with the wrong powder, What happens when you pop off the powder bottle and switch powders on a Mec? Is any left in the bar?

If there is, then both the variety, and quantity of powder could be wrong for the next shell.
I am not near my reloaders to see, so someone please see if this last idea is true.

I don't believe the shotshell has the available volume nor the bore/case volume and high powder charges, necessary for this type of event. But I could be wrong.

Shotgun powders are faster than rifle powders. A mistaken charge of a rifle powder in a shotshell is probably more likely to yield a squib than a high pressure...but you knew that. Now maybe some Bullseye pistol powder....

Bullseye is/was a 12ga shotgun powder, not all that disimilar to Red Dot in burn/pressure profile. However, it's unsuitable for laoding the 20ga, as are a half dozen other shotgun powders. Start with a common powder that's too fast for the 20ga to begin with and it's physically possible to double charge such a powder.

I think it's a likelier, and scarier, scenario with a 20 than a 12.

I'd put my money on obstruction, given the absolutely incredible pressure and short rise time that it musta taken to do that damage.
