I think Keith is on the right track ,the push button is usually strictly a set screw to attach the rod, just ahead of the slot for the bbl lug ,or a set screw and the rod attached to the button is threaded...this set up can and will back off over time ,ive seen it many times ,the face of that button should angle up or down if its cocked to either side you found the problem.
