Looks like an E.J.Churchill boxlock but I've never seen one with a lever bent that far. Maybe for someone with an arthritic thumb? I have the Churchill book and nothing similar in there either. Lagopus.....

I agree with Ohiosam. The lever is longer than normal and would hit the safety if it wasn't bent.

It may be the photograph but if you look closely the button on the safety also appears to be filed at an angle, almost parallel
to the contact surface of the top lever. Also note very full pistol grip and single trigger.It appears as if it was set up so the entire gun could be manipulated with one hand.

I think Hugh is right. That gun, including the stock head reinforcing, appears to have been originally made that way. Probably the owner had a need to primarily use his right hand when shooting.
