not enough clarity re "testing"

shells didn't fire first time but SAME shells fired
when gun was re-cocked?
or
shells didn't fire but others did?

handloads cause many problems
Try quality factory ammo.

if there are failures to fire examine gun
first correct feel and function as suggested

then chambers
including headspace
( with extractors or ejectors removed and then replaced)

if factory ammo fires correctly then you have reason to examine
ammo that malfunctions

thin rims
primers
worn shells ( especially primer pockets)

As I stated in the original post, I was load/pattern testing, inserting one(1) shell at a time.

Very well... understand now. TU.

I've been fooling with one here now for 30 minutes with snap caps and the time honored masking tape telltale.

If it's closed it shoots.

I can't duplicate Kutter's failure mode with this one, although I don't discount the possibility.

This is about as simple as a gun gets... sorry I can't be of more help.

WAIT!

It will too do it.

Move the top lever just enough so the safety won't come on.

Weak hits.

I'll be darned.

Nice one Kutter!

I believe that Kutter may be correct. I'm not in the habit of snapping my doubles shut. I ease them closed and then ease over the top lever, pushing the lever over if necessary, had one break that became a costly fix. Time will tell, I'll try "snapping" it closed in the future and see if I have the problem again.
